The ideal candidate will have the experience and personality to treat Geriatric Patients in their Main Line. Must possess a current SC Physician License, a driver's license, and an active DEA number. Doctorate degree, Required. No nights, No holidays, No hospital rounds. An average of 5-10 patients per day. More quality, face-to-face interaction with patients and families. Seeking full-time or part-time nurse practitioner / physician assistant providers. Responsibilities include providing primary services for patients who are homebound and need ongoing medical care. Responsibilities likely to include some hospice and palliative care as well. Individual must be currently licensed as a Nurse Practitioner in South Carolina, have ability to build rapport with patients and must be able to independently travel to assigned locations. Average census approximately 5-10 patients per day depending on location and type of visit.
Responsible for evaluation, planning, directing and administering physical/occupational therapy treatments as prescribed by a licensed physician. Administers treatments and physical/occupational agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability. Assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities. The Physical/Occupational Therapist coordinates, delegates and supervises responsibilities assigned to supportive staff (PTA, OTA). Must be licensed in South Carolina by the Board of Physical or Occupational Therapy Examiners. Must have valid driver’s license and able to independently travel to assigned locations.
